Kidney stones: The size of the stone does not correlate with the degree of suffering very well.  The extreme pain is caused by spasm of the ureter as the stone passes through.  Some  stones pass without causing ureteral colic pain.  The likelihood of passing the stone all the way to the bladder quickly is increased with 2-3 mm stones though.

Possibly: Stone pain is usually depends upon presence of blockege. There are also other causes of such back pain, i.E spinal disc, muscular, aortic anurism, pancreatitis etc. Best to consult your dr., or go to an er if pain is very severe.

If not, contact your prescriber for consideration of 2nd antibiotic. Have your husband drink lots of water to flush kidneys and if not already, discuss meds and/or diet to prevent further stones. Unrelated: What you are seeing is likely normal. Passing foamy urine now and then is normal, for the speed of urination and other factors can influence this. But you should see your doctor if you have persistently foamy urine that becomes more noticeable over time. This can be a sign of protein in your urine (proteinuria), which requires further evaluation.

Urologist: You should consult a urologist to ascertain if the stone is suitable for lithotripsy, endoscopic removal or would require open surgery. Once the stone is out, if the blood in urine persists, it would require and evaluation.
