Possible: You need to be evaluated.  You also need to be assessed for sexually transmitted disease.  Lastly, don't ignore the emotional turmoil this will cause-get counseling.  Also, even if you were intoxicated, you do not lose your right to say no, consider calling the police.

AIDS is NOT likely : If I am understanding your question correctly, it is unlikely for you to have AIDS if HIV testing is negative after 12 years since the rape. See your doctor for a discussion about your concerns.

Getting past the trigger will be difficult but make sure you don't neglect your own health. Self care is very important in any type of posttraumatic stress recovery. When you go, have a support person with you and find an empathic doctor. Your friends will know one.

Needs an evaluation: Bleeding is a sign of tissue injury. Neither self directed masturbation or sex should trigger bleeding. You will need to have the areas involved examined to find a source. There is likely a site that has not healed from that event.

Self induced pain: Self-induced pain to relieve mental or somatic pain is common. Apprarently one type of pain can block out another pain. Another reason is related to dissociation, derealization. Self induced pain helps some people get reconnected to reality and their own body. Rape is a traumatic event that can lead to mental anguish, depression, dissociation, etc. The post-traumatic stress syndrome is treatable.

ER: Sorry to hear that.  Please go to the er immediately and have them do a rape evaluation of you.  Time is of the essence.  They can search for particles, hair, semen, skin cells, etc.  You would also be suspicious if you have rectal pain or bleeding.  This is a crime and you need to go to the hospital first and then have the police try to catch this person before it happens to someone else.
