Gilbert's: The vast majority of folks like you have gilbert's non-disease. It's not a problem and the treatment (phenobarbital) will just turn a healthy man into a sick man. The entity to rule out, even if you're not anemic, is a mild hemoglobinopathy.

No big concerns but:: A slightly low TSH is nothing to worry about but ask your Dr. to check Free T3 and Free T4; they're more useful than total T4. Elevated bilirubin is almost certainly caused by Gilbert's Syndrome, a benign genetic condition, but it may help to minimize alcohol and stay well-hydrated. Rerun test: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, you need to re do the tests in non-fasting state. With the exception of elevated direct bilirubin, the findings are consistent with Gilbert syndrome, which is not a disease. HF: you have complex issues. Why do you have HF and also the cause of liver disease ? The AGMA is concerning and has to be worked up. Some of these conditions can be related. What hormones did you take ? I have more questions than answers, more so because you have 2 organ dysfunction and a acid base disorder and they can be related.

Hepatitis: Elevated ast and alt is consistent with liver inflammation (hepatitis). Hepatitis can be caused by liver disease, viruses, alcohol consumption and autoimmune diseases. Elevated bilirubin can be caused by hepatitis or obstruction of biliary tract. Hepatitis is needs to be further evaluated to determine the etiology.

Needs evaluation: Bilirubin is not affected directly by kidney failure or dialysis.  Various types of medication can cause elevation or a variety of liver and biliary tract (bile ducts) problems can cause elevation.  A level of 237  ( micro moles/liter not mg/dl) is high (normal is 2-17).  Have him discuss with primary MD and kidney doctor further evaluation and possibly seeing a liver specialist.

Liver injury?: Elevated liver enzymes indicate liver cell injury (enzymes leak outside the injured cells). Fatty liver disease (steatosis) can become more active when some of the liver cells can no hold no more fat so they rupture and thus add an inflammatory component to the process (it becomes steato-hepatitis)

Gilbert's : Hi. Your slightly elevated bilirubin is probably unrelated to fatty liver. Your bilirubin is probably mildly high because of an insignificant defect in bilirubin conjugation. Have your doc check total and conjugated bilirubin. With Gilbert's, total bili is high and conjugated bili is low.
