: The intervals cited may indicate polycystic ovarian syndrome, and ovulation may not be occurring.  See your GYN physician for an exam and additional testing (ultrasound). Ask about oral combination birth control to stop ovarian function.  If pregnancy is desired, see a reproductive endocrinologist for ovulation induction.

Sometimes Yeast: The most common cause of vaginal fissures is untreated yeast.  You can see your doctor and get tested for that.  Sometimes it can be hard to treat.  You could go to library and check out a book on yeast and see if it describes your symptoms.  Sometimes they are complex and difficult to treat, involving elimination diets and the like.  Without a photo, hard to diagnose, but that's a good guess.

Not always: Need free T3 (liothyronine) / free T4 levels and maybe even an uptake scan to completely rule it out.

TSH: Your TSH might be a bit high and the cause would be a decrease in the chemicals (T4 and T3) in the thyroid gland. Familial or otherwise, did your doctor tell you your TSH was high and did she/he recommend treatment?

Grade 3: Means that 50-75 percent of your breast tissue is made up of glandular tissue instead of fat. Nothing ominous.

Viral cultures: can confirm various subtypes of HPV. The bigger question of course, is how would sexual warts show up inside your anus? Get examined and tested.
