No problem: Intercourse generally does not cause issues with hernias unless you are having pain from the hernia in general.  Most positions are fine unless you are lifting her thus increasing intra-abdominal pressure.

Look for bulge.: Depending on the location of the hernia you should be able to note a fullness in the region the hernia is.  I would bear down as if you are lifting a heavy weight.  I would not rely on if the area is tender.  Your PCP should be able to help you.

Hernia reapir: is not related to the ability to become pregnant. Pregnancy could stretch out or cause a hernia recurrence in some. Ask your Obstetrician your concerns.

The smaller the hernia the greater the chance of incarceration of bowel, infarction and gangrene.

Most hernias: Occur spontaneously with no cause or reason. Sometimes hernias are associated with repetitive heavy lifting, obesity or past  pregnancy. Most common location of hernias are the groins or umbilical region. The cause of one type of hernia can be an incisionsl hernia caused by previous surgery.

Not usually: Though the reflux associated with some hernias will often feel like it is difficult to breathe.  Weight on the other hand can cause breathing problems and worsen the hiatal hernia symptoms.  Losing weight and increasing activity may help both of your problems.  Still, if they are not improving or worsening you should see an md.
