i've had hemorrhoids for 20 years and am 37, never been treated. now my anus is tight and i'm getting fissures and it's getting worse. is there hope?

Take a stool softener.my advise go and see  a colon Rectal surgeon for a proper examination and a proper diagnosis , treatment.

No: Hemmorhoids are varicose veins of the anal canal.  They occur externally where they can clot and forms nodular lesions or bleed.  Internal hemmorhoids are also associated with bleeding and may cause pain. Cuts in the region of the anal canal are fissures that are also painful and can bleed.

The  treatment though surgical has a very high success rate with mild pain in most cases. Also you should be checked for other possible problems such as rectal cancer or some neuropathy though the high probability is that your changes are from hemorrhoids.
