If : If the skin is tender to the touch then may he is suffering from an infection at the injection site. His leg feeling "locked up" is a worrisome sign. Maybe the injection hit a nerve, which is now partially damaged. Regardless of the possible causes of his symptoms he should be evaluated by his doctor right away or be seen in the er if he has a fever or worsening pain.
Lastly, to answer your question the cortisone injection typically takes about 24 hours to reach full effect.

More than the VMO: Strengthening of the vastus medialis obliquus muscle is only one part of the non-operative treatment of patellofemoral hypoplasia.  General quad strengthening, and lateral reticular stretching are other important considerations and i usually recommend at least a short course of formal therapy for patients to learn the correct techniques for these exercises.  Bracing can also be helpful.

Unclear: An osteochondroma is a benign cartilage capped growth that should not grow after removal. If it continues to grow or is causing new calcifications that is very concerning that this is not osteochondroma and may be something more concerning. You should certainly talk to your surgeon or consider an opinion from a musculoskeletal tumor surgeon.

Possibly: Asymmetry in people, absent specific life events to explain, is not a good thing. Especially with an associated issue (pain, numbness, weakness),  should see your dr or urgent care or an orthopedist for eval.

No: The femur would slip off the tibia if you major injury, also nothing will fracture unless you major trauma, having said that you stability is weaken by the ligaments injury so in your case less force needed to dislocated your knee or break the bone around the knee.
