Too heavy: The head is heavy and little babies (0-4 months) do not yet have strong enough neck muscles to control it so it sort of falls to one side. Gets better by 4 months. Older babies often do things because they figured out they can.

Details matter, of which we know none, such as why the hospital admission, what age is the baby, how was he growing prior to the admission, what condition he was in upon discharge, what he drank before, what he is drinking now, etc... : Could be and easy to diagnose. Have someone who knows how measure your corneal shape and depth  and  you will know.

Dreaming?: She should be seen, and have a sleep study.  It may be just having vivid dreams, even if she does not remember them, but a sleep study will help delineate if she has a problem.

Barring neurological: Disorders which a neurologist can examine you for, it may be a symptom of your anxiety disorder.  We sometimes associate comfort or security w/certain places or behaviors.  A therapist may help you move that rocking from your head into your much less obvious fingers through hypnosis or other strategies.  Peace and good health.

No need to worry.: However you should refrain from doing this frequently if you have known degenerative disk disease of the c spine.

Probably not: The medical community used to think that cracking the knuckles will lead to arthritis etc...Currently there is no concrete evidence to support that theory. So, cracking your knuckles, joints, back, neck without pain...I am not concerned.
