Re-evaluate: Tmj\tmd is not something that you generally just treat and it's cured. There are many contributing factors and episodes of stress, not wearing an appliance, gum chewing, etc. Can cause ongoing symptoms. When was the last time you saw your dentist for an evaluation? Are you following all instructions? See your dentist and perhaps an ENT specialist to rule out any medical issues.

Muscle Spasm: The description of your symptoms sounds like a muscle spasm in your upper neck.  The brain subconsciously contracts the para vertebral muscles to prevent perceived spinal injury. This is the reason, the pain become worse during the day. If the pain/spasm does not improve significantly, then discussing the symptoms with your doctor should be the next step.

In particular, what you describe is typical for a concussion, but you should be checked to be sure that you don't have a more serious kind of injury such as a subdural hematoma or a contusion.  Also, if you hit your neck, you should have an MRI scan of the neck to be sure you have not damaged your cervical spine and put your spinal cord at risk.  Do not delay in seeking appropriate evaluations.
