i have a male friend in his  fifties.  both his lower legs are swollen hard and are turning black what could cause this and is it treatable ?

Hard to say: As this most commonly would be related to vascular issues, possibly arterial or venous or both.  But his best bet is first see a doctor to better identify the cause, as it very well may not be related to anything previously mentioned, and to be able to better guide you/him, the actually cause of his symptoms are very important. ?Erythema nodosum: Hard to say without looking at them. Could be erythema nodosum which can be associated with many causes some of which are serious. You definitely need to get it checked by your doctor.

Genetics: Everybody's body carries fat in different areas preferentially. Yours is the lower extremities. Keep up with your diet and exercise and those areas will eventually lose their fat as well.

Swelling: How old?  Pregnant?  Both legs? Calve pain or tenderness ?  Problems urinating?

Few reasons: Is it possible your wife is pregnant? That could be a reason, sometimes medication side effects can cause this. Does she have varicose veins? Any issues with high blood pressure or cardiac issues? Does she eat a lot of salt? Any new finding such as the one you describe should always be checked out by your internist.

If it has been present for days or weeks, it could be caused by problems ranging from heart failure to blood clots in your legs to complications of diabetes. DVT or Phlebitis: This can be a very dangerous situation. If the redness and swelling includes your calves then you need to seek immediate emergency medical care.

If the rash and sores are just in the front of the lower legs this could be phlebitis. 

In either case you need medical attention. If you are having any shortness of breath or chest pain you need to call 911.

Difficult to say: Many causes of muscle pain.  Are you on a cholesterol medicine?  Potentially could be a virus such as parvo.  Could be other things as well.  I would recommend seeing your physician
