Most commonly: Either a hemorrhoid(dilated vein) or an anal fissure(a break in the surface tissue), but it's hard to say without examining you further.  You should try to optimize your bowel health to avoid hard stools that require you to strain.  Stay hydrated, eat fiber, consider using an over-the-counter stool softener in the short run like colace, (docusate sodium) consider sitz baths.  You should review this with your doc.

A good first step would be to increase the amount of fiber in your diet, including increasing the amount of liquid that you drink. Bowel Movements: I suspect that you are not getting enough finer in your diet. You want to include both insoluble fibre and soluble fibre from certain fruits and vegetables. It's also important to stay well hydrated.

If stool pattern has changed, is unusual for you, & the change persists, or is accompanied by other symptoms, see your doctor.
