It could from: Arthritis or injury happen to the joint. If it is from arthritis usually very close to the joint mainly on the top.

Scarring: After this surgery fibrosis or scarring sets in. Over a prolonged period of many months this should gradually  shrivel up and become flatter and less noticeable. If it becomes reddened, larger, tender, purulent or fluid filled, have the surgeon examine it.
