Treat it: The positive H. pylori antibody test is not a reliable indicator of current infection. The gold standard for H. pylori treatment is the gastric biopsy, and this is negative in this patient. However, I am impressed by the positive breath test and erosive gastritis.  I would have expected stool antigen to be positive, as it mirrors the breath test. Given the discrepant results, treat the patient.

Hard to know: I believe that the prevalence of H. pylori is much higher in Phnom Penh than in most of the United States.  Without knowing any specifics about the test that was used, I suspect the positive predictive value is high considering your location. This is increased given your history of gastritis. Treatment is straightforward and effective and should be considered.

Likely not: IgG to this organism points to a previous infection, IgM an active or very recent infection which is not the case for you. If you do have gastritis symptoms however, I believe a PCR assay from a stool sample would be the best way to detect it  if a culture of the gastric biopsy is not contemplated.

H Pylori Treatment : Following H Pylori treatment, the most important part of management is to know if a patient is having bloating, or pain suggestive of incompletely treated infection. Urea breath test is often done to determine eradication of the bug; IgG blood test is not helpful after treatment as the test may remain elevated for an indefinite period of time once someone is diagnosed with H Pylori.

How bad?: You either have an infection or do not have an infection.  I am unsure what you mean by "how bad' you have it.

Not now..: You have been exposed to H Pylori in the past. But you should not assume all these as 100%. false positive, false negative, wrong sample, wrong reporting etc matter.

Past infection: These results suggest past infection with H. pylori. The blood test can remain positive for years after past infection even if you were treated.  The stool test and urea breath test can better detect current infection.  The gold standard for testing is a biopsy during endoscopy.

Strange: I assume the doctor ordered these tests to evaluate your "heartburn," which is confusing to me. H. pylori is a bacterium that can cause chronic antral gastritis or peptic ulcer disease. I would have an extremely low pretest probability for either of these diseases in a 28 year-old. The breath test yields few false positives. The other tests are very sensitive, with very few false negatives.
