GYN/GI causes: At the age of 49, your risk of cancer can be increased and it's possible that you may have a disorder of your ovaries/liver that may be causing the collection of fluid in your abdomen, leading to your belly growing. I hope it's not the case that this is something bad, but you should really see your doctor about this.

Depends: If you're concerned that you might be pregnant, take a home pregnancy test.  They are inexpensive and accurate.  Best is to take one when you miss your period.

Bleeding/ bloated: Follow up with doctor/urgent care within 24-48 hours. Was a pregnancy test ever positive? Could be miscarriage, ectopic, or uterine bleeding from fibroids, polyp, mass  need sonongram or uterus and ovaries/ abdomen, Check urine

Just wait.: The important thing is how big are the babies---ultrasound should confirm adequate size. You should be gaining weight--with twins at five months about a kilogram every 2 weeks. If not , you need more calories!

See your Ob: At your pre-natal care visits, your Ob should measure your belly.  The # of centimeters measured should roughly equal the number of weeks. If this measurement hasn't become bigger, you may need an ultrsound to get more precise measurements.  It usually doesn't mean the baby is out of room to grow.  The belly is able to get bigger and bigger as the baby gets bigger.  Best wishes!
