Depending on your other health conditions and how well your Hemoglobin A1c (the measure of your sugar control over three months) responds to Metformin, your doctor may add on another medication to help improve your Hemoglobin A1c.

Glucophage : Glucophage or metformin is the most common diabetic medication world wide.  It is common that some people have short term diarrhea which resolves. In rare circumstances in some people with significant kidney issues, it can cause an acidic state in the body. In general, it is great medication I often recommend, and if I can, I  use it first with diabetics.

Not exactly: If you have a moderate degree of renal disease, metformin can cause lactic acidosis, which is very serious.  If you do not have renal disease, metformin is quite safe.

No problem: This medication is well studied in breastfeeding mothers and is considered safe for the baby. Negligible amounts (&lt;1%) are found in the breast milk and no effect on the baby has been noted.

See details: I try not to use an NSAID such as Mobic (meloxicam) in individuals with high cardiac risk. Individuals with hypertension, hyperlipidemia and diabetes are considered high risk.

Sexual Response: ED w/premature ejaculation is possible but not likely.  Continue being followed by a urologist and please see a clinical psychologist or licensed sex therapist.  You mentioned a medication, this is also something that has to be looked into as a possible cause.  For young healthy males, ED is usually psychological, too focused on "performance."
