Likely not pregnant: Plan B can disrupt 1-2 menstrual cycles resulting in later than expected periods.  It is likely that you are not  pregnant given the negative test in the doctor's office.  Follow up with doctor in one week if you are still without a period.

Reasons for missed /late periods: low body weight, obesity, marked weight gain or loss, over-exercise such as in endurance athletes, breast-feeding, stress, illness, eating disorders (bulimia or anorexia), travel, hormone irregularities, drug use, medications (i.e., birth control) or med problem (polycystic ovary syndrome or Asherman’s syndrome. #1 reason is pregnancy.

Preg test?: Obviously check a pregnancy test. Many factors could affect your cycle including hormonal changes, medical issues like hypothyroidism and stress. Follow up with a doctor for simple blood tests

I need more info: Has this happened before? Not unusual to have a delayed or minimal period. If this is not a common occurance and does not repeat you should be fine. If your health gets worse see your doctor!

To : To be on the safe side, you should take a pregnancy test.  If you still aren't having a normal period in 2 days after that, take another test.  You may need a blood pregnancy test as it is more accurate than a urine test.  The biggest concern with spotting is that it can occur with pregnancy, but hormone fluctuations can also cause lighter periods or spotting.

If you have pain, vomiting, or light-headedness/fainting, then you would need to be seen as an emergency.

"no periods" ?'s: At 45 , there are often times when periods are delayed b/c ovulation is delayed.  If pregnancy is possible, obtain a serum HCG to be certain.  Your cycles may resume on a fairly regular basis as the level of pituitary stimulation on ovarian function increases in "peri-manopause".
