Hard to say: You should ask the doc who ordered the Holter monitor this question. Normal people occasionally have ectopic beats. Whether or not your study would exclude such activities is best determined by someone who assessed you and the study.

Possibly: It is possible that cyano-cobalamine might be the cause of symptoms since it releases cyanide but methylcobalamine the preferred form, should not cause side effects. If stopping the b-12 supplement gets rid of your symptoms that would be evidence that the supplement is the culprit. Possibly you need other b vitamins for b-12 to work properly like methyl folate (folic acid) and b-6.  Look at your life style.

Not the case: Your symptoms are not from B12 toxicity. 1st, B12 toxicity is rare. 2nd, if you do happen to become toxic, then your symptoms would be numbness/tingling in the hands ; face, panic attacks, insomnia, and maybe rashes. If you already have an overactive thyroid or mitral valve disease, then B12 toxicity may make those worse. Dyspnea would result from worsening mitral disease but you'd lose weight.

Complicated: Normal 8 am cortisol is 8-23 micrograms per deciliter. Since you don't use units, it's not clear if you're normal or not. Moreover, even if it's low as I think you're hinting, your Symbicort (budesonide and formoterol) may have suppressed it. A cortrosyn stimulation test is needed. Finally, low cortisol could cause fatigue and low BP, but not the other symptoms. Clearly, you need to stay under your doctor's care!

See below: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, your TSH results on different days are not meaningfully different and are within normal limits. Your symptoms warrant an in-person evaluation by your doctor.
