Skin lesion: Anything elevated needs to be confirmed by doc as it may be concerning. Anything changing in color or size or causing any symptoms such as pain, itching, tingling needs to be looked at.

Freckle : Difficult to say since I don't know how long that black dot in middle has existed. Have your physician take a look. Usually - as long as there are no changes to any borders then usually not much concern.  However a trained provider must do a skin exam to determine if there another condition present.  Good luck

Seborrheic keratosis: If they are raised, they are likely skin tags or something called seborrheic keratosis (sk).  Sks usually turn up due to aging and they are benign.  They can itch, but its not a common symptom.  Its always a good idea to get checked out by a dermatologist.

Many Possibilities: Discoloration on your nail can be anything from fungus, to melanoma, to blood under the nail or just a mole. Some considerations to help determine what it is depends on the size, location,  and if  the "dot" is growing out. Best bet is to have a specialist eye it and decide if a biopsy is needed to differentiate the cause.

Floaters: A few dots are called floaters and are minor semi-opaque membranes and debris in the vitreous cavity.  If your vision is good, and there are no light flashes and you have not received something like a conk to the head, you are probably all right.  If you see hundreds of floaters all at once, then you need an evaluation fairly quickly.
