Torn what: Where is the tear?  Skin, muscle?  Skin lacerations are obvious but muscle tears are problematic.  The severity of the tear, the disability resulting, the area involved all play into the treatment.  See an orthopedist for an evaluation.

Biceps injury: The biceps tendon comes off the upper arm and inserts onto the proximal part of the forearm bone (the radius). During an arm wrestling event the biceps comes under tremendous load and this load can overload the insertion site of the tendon on the bone causing a significant injury or actual tendon tear. A thorough orthopaedic evaluation is now warranted.

is it normal for my hand to swell up 48 hours after a mid-forearm injury?

Depends: It depends on the type and severity of injury. If there is a lot of swelling in the forearm, this may impair venous return from the hand and cause swelling. This will be worsened if you are not careful to elevate the injured parts above the level of your heart for 48-72 hrs. If the swelling is associated with severe pain or numbness, or if it persists, you should see a hand specialist.

i injured my forearm lifting a heavy box 6 weeks ago. it is slowly getting better but there is still pain with some movements. should i see a doctor?

Perhaps: It depends on how much it is bothering you. If current pain is mild and you can do everything that you need or want to do, you can wait it out. If it is interfering with your ability to function, consider seeing your primary provider or a hand/upper extremity specialist, or a rehabilitation doctor.

i fell and injured my forearm near the elbow. the x-ray taken at urgent care came negative for fracture. three weeks later, i can still not fully extend my arm and it pops also. what should i do?

Need follow up: You can still have a fracture.  Radial head fractures can be difficult to see at first.  Because you are still having symptoms, follow up with your physician. He/she will like order a follow up x-ray.  If that is negative, an MRI may be warranted.  Hope you feel better soon.

i just had a rotator cuff strain injury from a fall.  can't lift my arm up.how long does it usually take to recover?  when can i resume playing sports

Best for you to get: Advise from someone who evaluates you, by examining/investigating you. He'll  inform you of the diagnosis/treatment.
So see an orthopod. 
It generally last for about 6 weeks, by then if its a partial tear/sprain, it should improve greatly.  A few patients don't improve and may need MRI and probably benefit from the surgical repair.
Returning to sports would be once the rc has healed.

are there any hand, wrist or arm fractures that cannot possibily be caused by a fall on an outstretched hand? i'm a mystery writer and need an injury that could be due to someone deflecting the arm of a gun-wielding perp.

Your : Your best bet is probably what is called the "nightstick" fracture.  If you need some technical help, i could help with the ways to distinguish a "direct mechanism" fracture from an "indirect" one.  While an ulnar shaft fracture can be caused by a fall, the pattern would be slightly different from a true "nightstick.".

Not enough info.: Depends on how long you have this pain and what brings it on. Unlikely to be serious

Possibly shingles: Often the early sign of shingles is painful skin.  Get seen right away if you see any rash developing.
