See your physician: It is common for 21 month old children to get viral upper respiratory illnesses which cause coughing and congestion. But no child during this process should be "foaming" at the mouth. That may indicate possible oropharyngeal swelling (throat) and difficulty swallowing. Please see your pediatrician immediately.

Slower...: ...To swallow saliva during sleep, and bubbles of saliva form and pop on the lips - wouldn't call it "foaming, " though.

Evaluation : Please see the emergency department right away. You may have had a seizure. They may order testing and imaging.

Does not sound norma: That is certainly a possiblity and there are other "parasomnias" that can cause this behavior.  This is easily clarified with a sleep study which a primary care doctor can arrange.  During a sleep study brain waves and patterns are measured and epileptic behavior would likely be detected.  It would also be noteworthy to keep a log before any appointment as to how he feels after sleeping like that.

Not a pseudoseizure: Generally, pseudoseisures do not usually occur during sleep.  A seizure can cause foaming and blood in the mouth due to biting of the tongue or cheeks.

Seek Immediate Help: These symptoms can be early signs of an allergic reaction or from aspiration/choking on a small object that is stuck in the airway. Another cause would be croup. Please seek help immediately to rule out the worst.

ER: Get to the emergency department as soon as possible. It sounds like you had a type of seizure . You should be evaluated by a neurologist and eeg should be performed.
You also could have experienced an allergic reaction . You may have has a mild aspiration while sleeping .Acid reflux can also cause wheezing and dyspnea if acidic material enters airways. If your breathing is difficult , call 911.

See your provider: You can get a referral to a sleep specialist or perhaps your PCP can help you with all of these including ways to get the weight down. It's possible to stop the problem - then you can ask them why. Best!
