Serous otitis media: Sounds like serous otitis media. A non infected collection of fluid behind the eardrum. This is why various antibiotics haven't worked.  May want to have an ENT doc look at it to confirm the diagnosis and consider treatment vs continued watching.

OME: You are correct that most cases do not respond to medications but the good news is that most will resolve by themselves in 2-3 months ( sometimes may even take longer). If you are too uncomfortable then consult your physician again after few more weeks.

Fluid vs. Anxiety: Yes, your dizziness can be caused by both fluid in your ear or anxiety.  Please start w/a visit to your doc to have your ears checked out.  If all is well consider seeing a clinical psychologist to learn some ways to manage anxiety.  Peace and good health.

Not usually: Fluid in the ear does not generally cause vertigo.  I've seen patients complain of imbalance with fluid in the ear.  The most common causes of vertigo are meniere's disease, migraine, benign paroxysmal positional vertigo (bppv), viral labyrinthitis, vestibular neuronitis.  Fluid is not seen in the middle ear with any of these, generally.  Fluid changes in the inner ear cannot be seen on exam.

Ear infection: A swimmer's ear will cause sharp ear pain and drainage of fluid. A middle ear infection with a tympanic membrane perforation will also produce that.

Accepted Answer

No: Nasonex (mometasone) works for rhinitis symptoms but has been shown to have no effects in the outcome of middle ear effusion. Effusion might increase the risk of getting middle ear infection. It may take several months before the fluid go away. Systemic corticosteroid may hasten the drainage.

Yes: In children, persistent fluid in the middle ear can resolve at any time after an infection; however, the longer it persists, the lower the odds that it will resolve on its own. Particularly if the child has recurrent infections, or shows signs of hearing loss, tubes may be necessary.

See doctor: Smelly fluid in the ear could mean an ear infection.  The pimply rash can be diagnosed and treated by the doctor at the same visit.
