Maternal ultrasound: Can sometimes already be detected after 10-12 weeks gestation. More commonly in mid to late pregnancy. Sometimes suspected and found in presence of oligohydramnios (reduced amniotic fluid).

Do nothing now: Baby is almost certainly perfectly OK. If renal pelvis 7.5mm, nothing to worry about &amp; will 99 - 100% settle down as a normal kidney. 7.5 cm is very large &amp; will almost certainly require intervention. Will require:- pediatric urologist, post natal US , VCUG &amp; nuclear scan after birth to evaluate swollen kidney for function. Removal only if no or essentially no function. Life with 1 kidney is OK.

See Pedi Urologist: Bilat hydronephrosis can indicate several different problems.  You should involve a pediatric urologist promptly.  In the male fetus, this can indicate posterior urethral valves which needs to be addressed immediately upon delivery.  It could also indicate bilateral vesico-ureteral reflux or obstruction where the kidneys drain into the ureters (upj-obstruction)  either way, see peds uro stat.

Hydronephrosis.: Hydronephrosis is a greek word for water in the kidneys; it is a medical term used to descibe abnormal urine retention inside the kidney collecting system, usually resulting from a stricture/ureterocele downstream in the ureter and/or bladder. If unilateral it is not serious, but bilateral can become a potential threat to the fetal health/life. Neonatologists will put these babies on amoxicillin.

Unilateral means that hydronephrosis is only present on one side.

Involves excising obstructing segment & rejoining ureter to kidney. Hydronephrosis due to high grade reflux or lower end ureteral obstruction may require uretero-vesicoplasty surgery.

Not at all: There is a very slightly increased risk of Down syndrome when prenatal hydronephrosis is diagnosed at maternal-fetal ultrasound during pregnancy.
