Depends on situation: If a woman has no gynecological problems: no probs w/periods, not sexually active, doesn't have pain, or need birth control - they may not see their gynecologist until age 21 (when the first pap smear is recommended now). But for any problems - they can see a gynecologist - at any age, for evaluation and treatment.

Depends: Most recommend pelvic exams by 21 or within two years of starting sexual activity.Some peds/adolescent med/fps provide these services as part of ongoing care of their patient population. Any sexual activity  requires surveylance for std's. Your primary care provider may refer you for specific consultations when necessary.

Genetic disease hunt: Congenital adrenal hyperplasia is a family of several illnesses in which the adrenals can't make enough cortisol without being pushed hard and then make other hormone as by-products. In the two common ones, 17alpha hydroxyprogesterone is up, in another it's absent; they're quite common and manageable if diagnosed & will make your life better.

Too vague: It's difficult to answer your question as it's very vague.  A more detailed description of the pain (what part of the body, menstrual cycle, how often, how long does it last, other symptoms, pregnant, etc.) would likely yield a more helpful answer for you.  Of course if the pain is severe, do not wait for an answer here and seek evaluation.

Not necessarily: Having a pap smear should not increase the moistness of your vaginal area, unless your simply feeling the results of the gel/lubricant the dr. May have used to help insert the speculum.

Gyn eval: The Obgyn can do a complete exam and address your irregular periods.  A gastroenterologist can address your chronic nausea. The PCP will need to continue to evaluate your fatigue

Usually: It is the practice of most ob/gyns to do a pregnancy test as part of an annual exam.

i have a mole on my lady part.. the outer labia i guess is the term. should i talk to a gynecologist about getting it removed or a dermatologist?

Either is fine: Either one should be able to remove it for you.  The gynecologist may be more comfortable working in that area and the dermatologist may have a better idea whether it is something you really need to worry about.
