Dysphagia: Sounds like you had or have scheduled a barium swallow.  That's a great first step for this problem.  If that doesn't provide an answer, get a referral to ENT for 'laryngoscopy' of throat and possibly 'esophagoscopy' of food passage.  Really can't say what is causing this common symptom without taking a look.

Phobia: This is most likely a symptom of anxiety. Since you have a phobia about choking, first get an examination by an Otolaryngologist for reassurance. If there is still this anxiety, an evaluation and instruction by a speech and swallowing physical therapist at your local hospital should help you overcome this fear.

Stuck in throat: Could be post nasal drip or reflux or excess mucus from asthma like reaction to cold. Consider zyrtec for post nasal drip, oemprazole for reflux , and follow up today with doctor/ urgent care may benefit from inhaler.

Possibly; see an MD: I recommend seeing an md asap.  The epiglottis can indeed become infected, and at worst can become so swollen that it obstructs the airway.  If it is infected, it is usually very treatable with antibiotics and steroids, but you need a proper diagnosis first.

Get an exam: It sounds like you're otherwise healthy, but it's concerning set of symptoms.  Needs in-person evaluation. You should have an exam to rule out something with your thyroid, or a herniated disc in your neck.  I would see someone within a week or two if you can.
