Low estrogen: Right before your period, your hormones drop to their lowest level (triggering the period), and so your vagina may feel dry and be more susceptible to irritation/pain. It'll pass as your menses finishes. If it remains a problem though, please see an MD.

Longer foreplay: Sounds like not enough lubrication.  Tell your partner to slow down and play with you longer before penetration.  Additionally lube can be added via theky line of products.  If these simple fixes don't work, then you need a gynexam.

Could be: If you had an affair outside of your relationship, you should be tested for STDs. There is no way to know what is causing your girlfriend's vaginal burning and urinary frequency without doing an exam and testing on her but if you have chlamydia there is a good chance that you have given it to her and you both need treatment. Honesty is the best and safest course even if it is the most difficult.

Muscle spasm: Some women will feel a vibration deep in the vagina. The exact cause is unknown but thought to be related to temporary vaginal muscle spasm similar to when one has eye twitching. It is typically a benign issue that resolves on its own.
