Enjoy a little honey: and fruit, but in moderation.  They are a close second to table sugar and that's the one that really needs to be avoided.  Also, studies show that diet soda can actually make us gain weight.  Use Stevia as a sweetener and avoid chemical sweeteners.  Peace and good health.

Indefinitely: Ideally, maintain a low fat healthy lifestyle. The fatty liver can recur, and you say he also has NASH, so best to keep liver as healthy as possible otherwise.

60 grams QD or so: No sugar, keep carbs to about 20 per meal. This is the equvialent of a slice of bread or an apple, 1/2 cup of rice, 1/2 cup spaghetti per meal
fatty liver is caused to a large degree by the production in the liver of triglycerides in the process of storing sugars. Fructose is the worst of the sugars because it cannot be directly burned in the krebs cycle must first be stored as trig by liver.

Fatty liver: Hormone therapy may contribute to fatty liver . Do you have any other medical conditions or any meds you take which may contribute to the  complaint i would continue to monitor fats in diet  and limit your worry for now.

Need further testing: You are absolutely correct, pain in your back and a generalized bad feeling after a fatty meal may mean there is a problem with your digestive system.  Given all of your symptoms it would be best to see your primary care doctor for consideration of more blood work and possibly further testing.

Many options...: There are many kinds of liver problems including some that cause excessive accumulation of metals eg. Copper and iron.  For those disorders a diet low in those minerals(copper and iron) is recommended.  For almost all other types of liver disease there are no specific restrictions.
