Exercise.: You need to exercise, and stay nutrtionally health.  Many women give up physicial activity, which then deconditions you, making everything strenuous. Get plenty of sleep. Take your prenatal vitamin. If problems persist, have your thtyroid checked by the OB doc!

Specialist: If you primary adrenal insufficiency, you will need to correct it before pregnancy.  Untreated, maternal mortality in pregnancy can be as high as 45%.  If treated appropriately, you shouldn't have a problem, although you will need Hydrocortisone at delivery, along with adequate saline hydration.

Fatigue: How close together were your pregnancies.  Be sure to replace all the nutrients you lost to your babies and treat any iron deficiency or endocrine problems e.g. Thyroid problems .  Somehow try to get enough sleep to keep from being sleep deprived.  Take good care of yourself and try to get help with your kids.  Maintain a healthy life style.

Yes: Pregnancy causes an increase in Progesterone which can result in a tired feeling until the body adjusts. Usually much better in second trimester.

Not much: Tsh, fsh, and lh all share a common "arm" as hormones produced by the pituitary gland. However, while there is some cross reactivity, pregnancy is much more complex than just a thyroid hormone alteration. You will still need to be on replacement (synthyroid) therapy, but it will not affect your pregnancy per se.
