Warning signs are chronic headaches that suddenly change in quality or intensity, multiple headaches a week, new onset headaches that affect one's ability to function, associated vomiting, associated dizziness/double vision/neck ache/fever/weakness, or the need to take daily pain medication.

Hard to know: The description is consistent with sinus pressure.  Inflammation/pain can nonspecifically be related to neck stiffness and fatigue.  There are many causes for sinus diease (allergy, non-allergic irritant exposure, temperature/weather sensitivity, structural problems, infection and more).  Discuss with a physician practiced in treating sinusitis/rhinitis to determine causation and treatment plan.

If you are using antihistamines to get relief, your fatigue may be a side effect of this. Sinus infection : It sounds like you have a sinus infection. Flonase will not be enough. You need an antibiotic that is very effective for the sinus like Augmentin (amoxicillin and clavulanate) 875mg two times a day for 2 weeks. You also need an sinus decongestant like claritin-d 12 hour also twice a day. On top of that you should use a saline nasal rinse like the neti pot several times a day. Once things settle down then the Flonase will work.

Pseudotumor cerebri: Is a condition characterized by raised intracranial pressure.  The differential includes venous sinus thrombosis, hypercoagulable state, obstructive sleep apnea, endocrine disorder, vascular malformations, and previous scarring from infection/bleeding. Tension: If it's a steady, dull, muscle tension type headache, tylenol (acetaminophen) or Ibuprofen usually help. If it's a throbbing migraine type headache, other prescription type  medications may be needed and you would have to see a physician for diagnosis and treatment.
