Patellofemoral issue: The likely cause of your condition is that the knee cap is getting off track when you bend your knee due to lack of quad muscle strength (VMO).  This causes the knee cap to track laterally when you bend your knee and leads to cartilage irritation (chondromalacia) under the knee cap which can cause aching and produce crunching sounds.  Keep working with your physio to correct the problem.

You can also consider an over the counter anti-inflammatory medication. Seek medical evaluation if the swelling and pain do not subside in several days.

Deep or superficial: if superficial can be bursitis. if deep can be in the quadriceps (for example tendinitis), or behind the patella (like chondromalacia or arthritis.

See orthopedic surgeon if pain persists despite conservative measures.

Yes: The kneecap or patella is very important for appropriate knee motion.  If its a serious fracture in multiple pieces, then i would seriously consider surgery.  

It generally will heal much better if the pieces are put back together!  its much more complicated but the final result is very important--poor healing will lead to even worse arthritis and more problems down the road.
