Strength: Strengthening muscles can be helpful and postural supports.  Unfortunately bone is stronger than muscle and this cannot be reversed but you can use your muscles to try to avoid.

Dowager's Hump: A dowager is a older woman who is dignified.   Older women often had a kyphotic spinal hump, thus came the term dowager’s hump.

If the latter correct your posture.
Pelvis thrust forwards, spine straight, head forwards, shoulders straight down.

Dowager hump?: To have a Dowager hump you first need to be a dowager. At your age that is not you.  Discuss Buffalo hump with team and Google images and see if you look like that.  Then discuss again.

See below: Work with a physical therapist or a yoga therapist. They can help you learn what good posture looks like and what exercises you need to do to attain a better posture.Once you get the "feel" of good posture, repeated attention to this will change your old patterns over time.

Neck: Lipo can be performed it is usually caused by steroid use, high chance of seroma as well as wound dehiscence 
i would recommend going to your internal physician to get checked out first.

Hard = bone soft  =: If you are discussing the curvature  of your spine - this is bone if  it is hard vs a lipoma or  other soft  tissue hump if it is  soft. Your physician can clarify. If it is the spine agree  it could be arthritis, osteoporosis. Your physician can help clarify and discuss the evaluation.

can i correct my scheuermann's disease with only exercises ? is that possible? i have only 48 degree of it and im 21.

To be honest: At age 21, it would be rare that you haven't reached musculoskeletal maturity.  Which mean's bracing isn't going to be of help and exercise, likewise, will not correct the physical deformity.  If you have pain, exercise may help with pain.  Otherwise, unless you have neurologic deficits or bothersome pain, surgery isn't routinely considered - it is also considered when there is more than 70o)
