Depends on cause: Trauma is the more common cause for subdurals.  Bleeding disorders are another common cause.  Anticoagulant use may lead to subdurals.  In rare cases no cause can be found.  Recurrence depends on the cause and the awareness of risks.

When veins that go between these layers rupture - often because of trauma - blood can start to pool between the dura mater and arachnoid mater causing a subdural hematoma.

Trauma is the most common cause.  In the elderly, even relatively minor head trauma can cause a sdh.  Surgery is often used to evacuate the hematoma.
