Mental health: An erection is more than 50% about your mind than about your body. If your penis works as you describe then you are either worrying too much and setting yourself up to fail, or you are not attracted to your partners.

You may need to get some therapy about this or find a way to get your mind out if the way.

Foley catheter: Assuming you have an indwelling Foley catheter for some reason?  It is uncomfortable to get an erection with a catheter in place.  Hopefully you will only need the catheter for a short time.

see a dr.: You have what appears to be an erectile dysfunction (ed), and having a good history and physical, as well as assessing for low testosterone, and abnormal prolactin levels needs to be done.  BP needs to be checked, diabetes ruled out, etc...Treatment may include ed drugs like viagra, levitra, (vardenafil) cialis, etc.

If masturbation: hurts, then you should not.   Otherwise, you can masturbate as much as you want without doing damage.   Masturbation does not usually cause impotence, but if carried out to climax, will have an effect on libido (lowering it, presumably).   You seem to rely on this behavior to accomplish other things--lowering anxiety?   Dealing with OCD?  Do you have a sex addiction?   See a doc.
