Inflammation: The epididymis is a  convoluted tube in the form of a structure behind  the testicle. Symptoms are pain and tenderness and swelling in that area.

Exam: testicle pain is a symptoms. Certainly testicular swelling with heat and redness is a danger sign. Avoid masturbation and sexual intercourse. Get examined- this week.

Not clear: I have seen multiple cases of epidydimitis. I have never seen a case of fungal epidydimitis  and feel this is only in someone that might be immunocompromised.  If you feel this is something you have then you need to see a urologist.

Doubt epididymitis: Even with a "thickened" epididymis with tenderness, epididymitis is unlikely. Most epididymitis causes enlargement and severe tenderness of the entire testicle. That said, if it feels abnormal, see a doctor. At your age, almost all epididymitis is due to gonorrhea or chlamydia. If tests for those STDs are negative, an no penile discharege, epididymitis is unlikely. Follow your doctor's advice.

Low sperm count: Possibly but not likely. Oligospermia (low sperm count). Can be due to previous testis infection or epididymal scarring from previous infection and inflammation, however most men who have had epididymitis or even orchitis do not subsequently find themselves sub fertile or infertile.  See a urologist and have a semen analysis. Ed is definitely not related.

: Most epididymal cysts do go away on their own, but it's important to be sure it's the proper diagnosis and not a tumor. I assume you've been examined by a urologist and an ultrasound of the testicles has been performed.
