Maybe: If it is a recent occurance, it may be a hernia.  It may also be a hydrocele, varicocele, infection, etc.  It should be evaluated by yout doctor.

inguinal hernia surgery 3 mths ago. left testicle is still enlarged. is this normal? what could it be?

May be normal : Firstly, it is normal to have testicles that are different sizes. Reasons why one could appear to change size is fluid around it, that could be the result of a normal reaction after inguinal hernia repair. If its still bothering you or enlarged after six months, see your surgeon for an exam. Hope this helps!

Swollen testicle: The urologist is right
the swelling is because that part of your body is beyond the surgical site the usual drainage is disrupted.  Remember this all hangs down when u stand up

give it time.

Internal ring tight: Consult you surgeon immediately.  
Till then wear athletic supporter, lie down as much as possible with legs elevated and ice packs.

Hernia repair: Depends what kind of laparoscopic  hernia repair you had.  Distended veins may represent blood clot. May benefit from ct angiogram.

No, ...: There is no reason for inguinal hernia to make testicle hard. However, long-term hernia sac may be felt hard and looks large leading the laypersons deceptively think testicle looks hard. So, ask Doc -surgeon timely in detail.

Should not have: pain from normal hernia unless complications sets in, You testicular pain may be unrelated see your doctor and get your hernia repaired , is a low risk out patient  surgery.

A mass could be due to swollen or hardened normal contents, growth of new tissue (such as a cancer), or the accumulation of fluid in the scrotum.  A doctor can initially use non-invasive testing such as ultrasound to help evaluate the cause.
