Maybe....: You've asked a number of questions to healthtap recently regarding the subject of "fatty liver."  fatty liver is common in america (about 1/4 adult population), is typically seen in association with obesity, high cholesterol, alcohol excess, and diabetes.  Distinguish steatosis (normal liver function tests and no inflammation) versus steatohepatitis (elevated liver functions with inflammation).

Some other causes..: Fatty liver disease is most often the result of excess alcohol and/or obesity, but some other causes are: diabetes, abetalipoproteinemia, glycogen storage diseases, weber-christian disease, acute fatty liver of pregnancy, lipodystrophy, inflammatoty bowel disease, hiv, hepc, malnutrition, alpha-1-antitrypsin deficiency, and many meds...Consult doc to search for your cause. Fatty liver disease: A fatty liver can be the result of too many calories resulting in being overweight or obese, it may also be a result of taking drugs or toxins injurious to the liver.  Associated with Diabetes and high cholesterol.
Alcoholic liver disease also results in a fatty liver,  caused by over consumption of alcohol;a herald for chronic liver disease if alcohol intake is not decreased or stopped

It could.: I have certainly seen patients with evidence of fatty liver reverse the condition with diet and exercise induced weight loss, particularly when the waist circumference is reduced.  Fatty liver whether NAFLD or NASH are potentially serious conditions which can lead to cirrhosis.

i had an ultrasound which showed a enlarged liver 20cm and a enlarged spleen 14.4cm both showed fatty infiltration. what are my doctors next steps and is my liver already damaged?

: Not necessarily. If your issues are due to fatty liver, it can be reversed. Losing weight is essential. (Fatty infiltration of the spleen is unlikely.) Increase in portal pressure due to liver disease is a more likely cause of splenic enlargement. It would be prudent to consult a liver specialist.

No answer: We believe the fatty livers due to being overweight however that's not the case all the time we know many individuals who are not obese but still have abnormal livers however it is absolutely a certainty that if you're overweight and chances are you'll have a fatty liver therefore losing weight can only help the situation as always please use exercise proper nutrition and diet is your appropriate rules for living a good life.

: The treatment usually involves addressing the possible cause or causes for fatty liver. Avoidance of the typical offenders, such as alcohol, supplements or OTC drugs would help in all conditions.

Liver disease: Liver disease can be caused by a variety of factors, including diet, medication, alcohol, genetics, and infection (e.g. Hep C).  Fatty liver disease can be potentially reversed if you are able to treat.  Fatty liver and enlarged liver are not necessarily the same.  Cirrhosis is thought to be irreversible.  Cirrhosis often starts with fatty liver disease.  Seek help for heavy drinking.

Slow process: It is a slow process but the key is to be persistent in losing weight. Remember the diet has a major role to play. U should have ur primary care doc check your liver function test as that is a good indicator of recovery.

i've had elevated alt (75) for 5-6 months and ultrasound showed a mildly enlarged liver (16.6 cm) but no fat. is there a chance this could be serious?

ANY abnormality: of the Liver function has "serious" possibilities  and "benign" reasons...for example "glycogen storage disorders" can cause the abnormalities you mention and many of these are benign!
You need evaluation by a Board Certified Gastroenterologist!  Ask your PCP (Primary Care Provider) for a referral.  Hope this helps and good luck!
