why do i have engorged veins on the sides of my head when i workout?

Positional change.: The veins in the face just as the veins in the rest of the body change in size depending on changes in position as well as changes in levels of hydration and changes in pressure in the chest and abdominal cavity. In the face, the veins decrease with standing and increase with lying down.  Straining or laughing increase the the pressure in the chest which can increase the size of the facial veins.

my varicocele was embolized in the main bore of the gonadal vein, 1" from the renal vein entry. will the engorged veins below die off and drain through other veins or could they remain active/dilated?

Unknown: I would think anticoagulation of some sort would be considered if you were significantly symptomatic. I would defer this long term radiologic finding to your urologist.

left arm pain, with intermittently engorged veins ongoing for last 4-6 months. duplex ultrasound of arm clear. (ive also had a few ivs in 2021) what could possibly be causing my issue? soon getting an emg of left arm, and then an mri of arteries?

: Seems like a lot of tests. What are they looking for? Where is the pain? What type? What makes it better or worse? You need a good history and physical exam to be co-related with the needed tests so please find a good MD or vascular surgeon to do that. Good luck

May be normal anatom: Large veins are not uncommon under the tongue. To check out your veins requires someone with experience to look at you--usually a dentist.

Not thrombophlebitis: It is normal for veins to expand and contract in relation to their relative position to the height of the heart.
Throbophlebitis is a clot in the vein  and such a vein would not change with the height of the arm

when a varicocele vein is embolized, can the same vein below the embolization coil continue to try to pass blood back to the heart?  can this cause the vein to remain engorged and for how long?

Varicocele: In general, the closer to the testicle that a varicocele procedure is performed, the greater chance for success. Embolization may be minimally invasive but it has a higher rate of recurrence than other techniques. If you are not satisfied with the result, I would consult a urologist who performs subinguinal varicocelectomy using a surgical microscope.

Depends: Hypertension a common cause, but inflammation / mass not impossible--depending on why you are getting fundoscopic exam. Discuss w/your dr so do not worry needlessly.

Not normal: Engorged blood vessels in one or both eyes is concerning.  It may represent a tumor behind the eye compressing blood vessels, glaucoma, thyroid eye disease or blood vessel disease.  This is something that should be checked out right away by your ophthalmologist.

Get seen: We need to rule out a mediastinal lymphoma, a cardiomyopathy that requires steroids to stay in remission, and a few other entities. You should be commended on your powers of observation.

Flow: The portal vein runs between your stomach and your liver. When you digest food the basic products are collected in your stomach circulation and delivered to your liver by the portal vein. Regular flow is 20 to 40 cm/s.  This is measured by ultrasound. The ultrasonographer takes a cross section of your portal vein and are able to measure flow. You are flowing, but your rate is slower at 9.2 cm/s
