Varies by state: Some support for service dogs is available in ca but I do not think they cover the cost of the animal.  Several community groups provide the animals to people with disabilities free of charge.

No: Service animals are almost exclusively dogs (w exception of miniature horses).
Emotional support animals are not restricted by species (dogs, cats, rabbits, ferrets, miniature pig, monkey, snake, hedgehog, rat, mouse, bird, etc.).

Yes: It is called an emotional support animal, usually a dog. Not everywhere is accepted, but a certificate from your dr may help you gain access in places not normally accepting animals.

Great idea: I love dogs, and service dogs are magnificent.  All large communities have these available, and you can customize based on needs.  Curiously, there is an incidence of epilepsy in poodles, but other dogs such as labs or shepherds may be a good choice.  If your seizures are tough to manage, and meds not quite enough, inquire about vagal nerve stimulator units.

Ask Your Dr. : An emotional support animal requires that you have a condition that causes you to have a significant disability that the animal helps you with. An example would be agoraphobia or panic attacks and the dog allows you to go out in public. If you have depression it  might be helpful to have a dog but it certainly does not require you to take it in to a restaurant with you.

Therapy Dog vs: Emotional security animal vs working service dog.  Ada does not deem an emotional security animal to be  a working service dog. A therapy dog is not considered a service animal or an emotional support animal.

Dr. exam, then note: Generally, doctors do not write work-excuse or school-excuse notes, or back-to-work or back-to-school notes, without an exam. That is because a doctor's note is his written, signed, professional opinion, on whether a patient can or cannot do a particular activity. An in-person exam gives confidence to the doctor's opinion. A highly-trained, truly certified service animal should be no problem.
