: Depends on how much smoking and for how long.  No real “tar” in bronchi or trachea but there are abnormalities of bronchial and tracheal function plus deposition of various carcinogenic (cancer producing) materials.  These clear at various rates - earliest after several months, latest over many years.  Other effects (poor circulation) may also take years to correct

Effect of smoking: Smoking will tend to decrease your sense of taste, irritate all the soft tissues in your mouth in general and can cause cell mutation resulting in oral cancer. It still escapes me why anyone would want to smoke knowing the documented dangers to your body.

Yes: It is quite difficult to tear a trachea because it is both tough and protected.  A fear would allow all kinds of the wrong things directly into your airway and would require surgical repair.

Aspiration: In an otherwise healthy adult, aspirating a small bit of food or liquid will trigger a strong cough which should expel it.  Aspiration pneumonia is quite infrequent in healthy adults.

Panic, seizures,: Psychosis, dissociation, racing thoughts, racing heartbeat, etc.  Severe anxiety and psychosis can persist when the drug/s are stopped.  Spice is a generic term for many different synthetic cannabinoids; they affect the same receptors as marijuana but are unpredictable in their effects.  The drug/s are very addicting and often unpleasant in effect.  It's unknown if they affect women differently.

Nothing serious: Nicotine is addictive but it tastes bitter so most folks do not like to take it for too long. There are no other ill effects that i know about.
