Variable: The cost would depend on the extent of the damage and where the repair was done.  Repairs in a hospital surgical suite will be much more expensive than those done in a physicians clinic.

Simple: One of the most common procedures performed.  The surgeon freshes up the ear lobe under local anesthesia and then sutures it back together.  Re-iercing  is  redone typically not on the resultant scar.  Done properly, the scar is inconspicuousl.

Good question: I can't say it is normal or abnormal without knowing what the surgery was for, the extent thereof, the way it went, etc. The best way to get your answers would be to call your surgeon (sorry to disappoint you with that kind of answer), but that's bad medicine to reassure you without knowing details.

Not likely related: The nerves that were blocked and the nerves dealing with your ear are not the same. It is unlikely that the anesthetic block is the issue. You may have been laying on that ear to cause your symptoms, or they may be a totally unrelated issue that have occurred in time suggesting a relationship where there is none.
