Return to doctor: or an urgent care center. You do not want the surgical site to get infected.  If the bandages are not changed properly that is a possibility.  Sounds like the surgery went well.  Peace and good health.

Not necessarily: Ear lobe crease is often observed in people with heart condition but it is not a cause or effect . It's presence does not necessarily mean that one has a heart condition or will developed one.

That might: just be his skull bones that are more prominent, presuming there was no head injury. Skulls come in all shape and size. Have it checked it concerned.

Seborhhea: Red cracked skin behind the ear fold is often an irritation of the oil glands. Try gentle soap/water cleaning - pat dry and apply an otc steroid cream. IF there is a heavy yellow crusting over the area it might be impetigo - a touch of neosporin may help. If this continues to happen have your MD confirm what the cause is.

: Just a pimple or a boil, you can put warm compress (10 minutes) for soothing the pain 3-4 times next 24 hours.

Scales: Dermatitis, possibly contact, allergic. Stop any alkaline soaps, alkaline shampoos, hair sprays. Apply OTC cortisone cream 2-3 times /day

Lymph node?: A lymph node resides in this area synonymous with mononucleosis  (mono / kissing disease ) If you are having other symptoms you need to be evaluated,  and possibly for lymphoma given your age and other neck pains.
