Metformin was approved for use in the mid 1990's and since then 9 more classes of diabetes medications have been added as treatment options including a new class just approved in 4/2013. Prior to metformin we had just Insulin and one class of oral medication (sulfonylureas). Many new insulins have been added since then as well.

GLP-1 and DPP-4: Some of the newer medications for type ii diabetes include the glp-1 agonists Exenatide and liraglutide and the dpp-4 inhibitors  sitagliptin , saxagliptin, and linagliptin.

For type ii, add glp-1 analogue slowly (victoza (liraglutide) has best track record), consider metformin. Most other dm meds lower but advance dm & problems, thus avoid.

For those who are recently diagnosed with type 2 diabetes, incretins (glp-1 agonoists) are showing promising results.
