Most likely fungal: This is most likely a fungal infection in this area. Try an over the counter antifungal cream like Lamisil (terbenafine) but if that does not get rid of it you will need to see a doctor.

If sudden onset,  seek er immediately for possible torsion (twisted testicle) that can impair blood flow and result in loss of testicle.
Other causes: infection, std, hernia, cancer, and benign disease.

Dull pain in left scrotum can occur with varicocele (feels like bag "bag of worms" in left scrotum when standing erect). Your pain could be related to urethritis, which might also be responsible for blood trace.
