Accepted Answer

Yes, and NO: Yes you can use Astelin daily, and no it does not cause rebound congestion, or rhinitis medicamentosa.  Unlike steroid nasal sprays, you don't have to use it daily to see an effect.  It works best using twice per day.

No : Rebound nasal congestion is most commonly seen with topical nasal decongestants such as oxymetazoline as found in afrin nasaly spray.  Antihistamines are not associated with rebound symptoms including congestion.

Steroids: I would recommend either Medrol dose-pak or Prednisone 20 mg. Three times a day for 5 days.  Prednisone is the ultimate anti-inflammatory drug and the above 2 methods of administration are safe and should relieve rebound congestion.  Obviously, stop taking over the counter nasal sprays.  If you have nasal congestion, see an ENT doc or an allergist.

Rebound: In general, oral decongestants like sudafed to not cause rebound congestion. Rebound congestion (rhinitis medicamentosa) is related to topical decongestants (sprays and drops).

Short term only: Afrin is only meant for short term use. If you need something frequently then you need to try other medications like nasal steroids and nasal saline. You should not use Afrin more than three days in a month. More frequent use may cause rebound congestion.

: Afrin contains a medication that reduces swelling by constricting the local capillaries. Over time with repeated use that trauma can cause the capillaries to leak causing the rebound congestion. Usually that would occur after 3 or more days in a row of use. Some may use a greater amount per treatment increasing the risk. Doing the minimal needed for improvement every other day may be tolerated.

can you take a monteleukast with a 2nd gen antihistamine, and nasal corticosteroid? would you be able to add an additional oral decongestant like psudoephedrine to this if decongestion is a concern? is there a risk of rebound congestion?

: yes/no. All of the meds cited work in different pathways to  provide relief. Rebound congestion is a factor with nasal sprays that include a med that constricts the blood vessels. Over time the constriction injures the blood vessels &amp; they leak, causing congestion. This is not a feature of oral meds like pseudoephedrine.

Accepted Answer

Normal: Up to 70% of the population has the nasal cycle which is alternating congestion as a result of changes in blood flow through the nasal tissues.  When we lay down there is an increase in nasal blood flow so that there is about a 20% increase in tissue thickness which will narrow the nasal airway.
