: If your testosterone is normal with a single functional testicle you are fine, Some young people does have low testosterone level after unilateral orchiectomy.
The things you mention do not affect testosterone level by my knowledge. However, gaining weight due to poor activity and lack of exercise may have a negative impact with testosterone production.

Many things: First issue is timing.  Specimen must get to lab within 20-30 min.  Hours old sperm swim poorly. It must be kept warm (cold sperm swim poorly). Medications and recent febrile illnesses can also effect semen analysis.  Talk to a urologist about reversible causes of sub-normal semen analysis.

Goal??: I think you mean boxers?  1st, are you and a woman having fertility problems (unable to get pregnant?).  If not, then your sperm count isn't an issue - wear boxers!  
next, plenty of couples get pregnant with the male wearing briefs.  If you cannot get pregnant talk to a doctor before you start worrying about your underwear.
Finally, a jockstrap would be just like briefs with reg to sperm count.

LOW TESTOSTERONE : Obesity with increased deposition of adipose tissue, tends to increase production of estrogen and decrease TESTOSTERONE .  THE Adipose tissue also absorbs insulin and increases risk of diabetes. I  would suggest a second urology opinion. Recheck labs Especially TFTS/TESTOSERONE level. Recheck testicular ultrasound. Consider empiric trial of viagra/Levitra/cialis. Also gluten free diet/wg loss.

: If you get a normal erection and ejaculate normally don't worry. amount of ejaculation can vary with frequency of sex. If you are satisfied with sex dont try to count contractions. Try that. But if you are still unhappy pl. see a Urologist
