Ovulatory pain: Some women get a little cramping when they ovulate this is called " Mittelschmertz" about 20% of women get this mid cycle for a day or so.  Some women can identify which ovary is releasing the egg, by the side the pain localizes too!   This is not harmful, and you can take regular otc meds as ibuprofen if the pain warrants.

Prostaglandins were originally found in the prostate gland of men.  We have them in other parts of our body as well.

Painful cramps: There can be a number of reasons for painful menstrual cramps without a period. I suggest that you do a home pregnancy test.  If negative and no menses repeat in one week.  If still negative and no menses i suggest a blood pregnancy test and a visit to your gyn.

It is superficial: You get cramps because the uterus is a big muscle and it is typically trying to squeeze out some blood.  The uterus is also located deep down in your pelvis and heat from a heating pad simply does not reach it.

Probably not: These are probably not menstrual cramps if it is only when you urinate. You should get a urine analysis to see if you have cystitis. You may need an antibiotic  and a pill  to soothe the bladder. See your family doctor or a urologist.
