This pain can be from the sheer size of the fibroids pressing on pelvic nerves to cause pain or due to heavy menstrual bleeding with the passage of clots through the cervix in to the vagina.

Tiredness: There are many causes of your symptoms.  A failure of your adrenals, Addison's disease, causes fatigue, but this is rare, and unlikely as a cause.  The term "Adrenal Fatigue" is popular on the internet, but is mostly a myth. Adrenals either work, or don't work, and you would need an ACTH test to figure that out.

The cervix can bleed from trauma or lesions in the cervix or cervical cancer, but it is not period bleeding. If you have had 4 episodes of bleeding in 8 weeks, you need to see a doctor for evaluation and treatment options.

Some: Some, but not a whole lot.  If indeed this is a complex cyst, these are common and benign.  In cases where there's ambiguity (when you see the word "may,") then depending on symptoms and sometimes lab values, this study would either be repeated in time to assess for change, or a more detailed imaging technique such as CT or MRI would be used to evaluate the lesion in more detail without waiting.
