Yes, both can: Complex cysts of the ovary and uterine fibroids can each cause pain. Pain from fibroids is often worse just before or during menses. This pain can be from the sheer size of the fibroids pressing on pelvic nerves to cause pain or due to heavy menstrual bleeding with the passage of clots through the cervix in to the vagina.
