"Gut-ripping" bad!: Scarring or adhesions from surgery can tear with activity, and it can cause pain, although usually not bleeding. However, pain described as "gut-wrenching" sounds pretty bad. If that is continuing, you should be seen so a good history and exam can be done to give you a better answer!

Scar revision: I assume you are referring to the horiontal pubic scar. Options depend on time since surgery and the scar appearance. Pressure, silicone, injections and/ or surgery are all options.

Scar tissue: Probably scar tissue or granulation tissue.  Probably nothing to worry about.  Hopefully tissue was sent to a pathologist for examination.

Back pain: If the surgery is recent, then scar tissue is far from the cause of the back pain. It is most likely the position of the  stirups during surgery that can cause some back pain after surgery. Especially if surgery was a long one. Anyone who has their legs in that position will have some back discomfort after surgery. If it persists, and over the counter meds don't help. See your physician for tremet.

Clean and protected: Cuts in this area should heal quickly with local wound care. Keeping the area clean (soap and water) and moist  (topical antibiotic ointment) and covered should do it. If not, there maybe a problem. Non healing wounds should be checked by a doctor to prevent further injury.

Needs exam : Having your uterus removed should not give you an enlarged abdomen. See your doctor to see what is going on. Perhaps you had a post operative hernia.
