i have a small lump on top of my right testicle for about 7 years now. it rests on top, but it is not attached to the testicle and i can move the lump around. it has not gotten bigger in all this time. testicular cancer?

Cancer unlikely: An appendix of a testicle (a small lump near a testicle) might be that small, mobile lump on top of a testicle. No lump growth in 7 years since age 14 makes it benign, not cancerous... unless it were to suddenly start growing. A college guy may not have gotten a routine check-up since high school... so this is an opportunity to call for a check-up plus specifically look at the testes and scrotum.

Always possible but: A guy in his early 20's is not likely to have cancer in a testicle, but testis soreness should not be ignored. Thus, his doctor should closely examine both testes and scrotum. Testicular Cancer: Testicular cancer strikes men ages 26 to 32.  It is not found in men of your age. If you are having pain it is most likely a problem with your tubing that goes from the testicles to your prostate.  This is common and is known as a varicocele or spermatocele.

Lumpy testicles: No. If you are 23 then is is most likely something else. Testicular cancer hits men between ages 26 and 32.  

If you truly have a lump on the testicle (rare) and not on the various tubes and blood vessels in the scrotum then you need to see your physician who very well may order an ultra sound to determine what this is.

Early detection of testicular cancer is important as in early stage testicular is a curable disease. If scrotal lump arises from testicle, further evaluation with ultrasound is performed.
