: You contact your doctor. Such excuses are legal documents that must stand up to court scrutiny. Docs who provide them to patients they do not know for issues they have not assessed can have their license restricted or revoked. If you want to pay for a virtual consult with A HT Doc and see if they will provide one go to www.connect@healthtap.com. Payment is due whether they provide a note or not.

Not here: We cannot send excuses for patients through this forum, see we don't treat you here.  If you have a consult with a doctor on Healthtap Prime, and an excuse is in order, it may be possible to do it there.

See details: Count back 40 weeks from the due date and you will know the approximate day of conception.

: Yes, that should be practical. Call your Jury office and ask if they would excuse you without a note from a doctor. If that does not work you can ask your doctor for a note addressed to the Jury office.
Do consult a doctor any way as your issue is treatable.

Consult summary: If you have used our Virtual Consult service, and asked the Dr for a note for work, the note would be included in the summary you would have received via email. If you did not ask for a note for work, then none was written and you will have to reconsult.

Generally, doctors do not write notes such as work-excuse or school-excuse notes, or back-to-work or back-to-school notes, without an exam. That is because a doctor's note is his written, signed, professional opinion, on whether a patient can or cannot do a particular activity. An in-person exam gives confidence to the doctor's opinion.

Work excuse : If you will give the doctor through this service enough information to make an assessment of your illness, a work excuse can be included in the note for that visit.

: Many times, these ads are intended to appeal to a potential group of new customers to increase sales.  There would be absolutely NO contra-indication of which I would be aware of, as you do not mention brand, but likely this can be used for men of all backgrounds.
